1. Applicants are advised to send their applications, passports, etc.- by Certified or Registered mail.
   
2. For returning the Passport by Certified mail please send a self-addressed stamped envelope along with your application OR include an additional amount of $ 4.00 in case of one passport and an extra $ 1.00 for every additional passport, to the fees towards return mail charges. If you want delivery by overnight mail, please enclose $14.00 extra by money order in favor of Embassy of India/Consulate General of India. Account numbers, Checks and CODs are not acceptable.
   
3. Passport fee once received cannot be refunded, even if the application is withdrawn or passport is refused.
   
4. Application forms should be filled in full, legibly (capital letters) and accurately and submitted with your passport. Incomplete or illegibly filled forms will be rejected.
   
5. Please attach a photocopy of applicant’s Resident Alien/Employment Authorization card.
   
6. In case of change in appearance, a Notary Public should attest a copy of the photo.
   
7. Your passport is a valuable document. It should always be kept either in your own custody or in the custody of a person duly authorized by you. If lost or destroyed, the fact and circumstances should be immediately reported to the nearest Indian Mission and the local police.
   
8. Your passport must not be altered or mutilated in anyway nor any endorsement made on it by any person other than a duly authorized official of the Government of India.
   
9. The maximum life of an Indian international passport is ten (10) years. Please ensure you obtain a fresh passport a few months before it expire, as issue of new passport, when the previous passport has expired is a lengthy process and could take a number of weeks depending upon the period by which the passport has expired.
   
10. Paste Passport size photograph on the first page of the application and enclose two more with the application. For lost passport, enclose six more with the application.
    
11. Emergency fee for issue of passport (after 10 years) on same day is $90

SPECIAL REQUIREMENT FOR LOST PASSPORTS

12. If you are applying for a new passport in lieu of a lost or damaged passport, please submit a letter explaining how the passport was lost or damaged along with the police report (in case of lost passports), and six copies of the application duly filled in and seven passport size photographs. LOST OR DAMAGED PASSPORT FEE: $ 200.00
    
13. New Passport Fee:
    $ 24.00 (32 pages & 10 years validity)
    $ 32.00 (60 pages & 10 years validity)
    
14. There is a separate form for change of name, birth/life/marriage/police clearance certificate and endorsement for emigration not required etc (miscellaneous services form)
    
15. Declaration in column 26 to be notarized in applications for replacement of lost passports or passports, which have not been renewed for over six months.
